Fixed duplicate merges on bursts
Rapid writes of the same fact within 200 ms could create two memories instead of one. Fixed.

The bug
If an agent wrote the same fact twice within about 200 ms, for example from two parallel tool calls, dedupe could miss it and store both copies.
The fix
Writes are now serialised per user for a 500 ms window. We merged the 0.4% of affected memories retroactively; you do not need to do anything.
